I had similar problem. If you have multimeter, then you can check rezistance on connector from temp. senzor for hotend. Correct value is 100k ohms.
I my case was about 1ohm (short) and it was caused by small worm in the hotend witch fix thermal senzor in position.

Yes i had problem on all 4 sides. Bottom and top side was ok. If you have side ribbing only on left and right side then is possible that is some screw poorly tightened on X axis, or low tension belts. But if you have problem with all 4 sides it is problem on Z axis.

You can set the offset or specific printing area in the repetier-host printer settings / printer shape.

Are you sure the extruder and hotend are properly assembled? When i finish my printer had similer problem. After disassemble hotend found distance between nozzle and throad.
