Yes, I get it now. My metal plate between holder and extruder was flipped the other way placing my fan more to the left of the bracket. I've changed it now and tried the fan duct. I have a feeling it blowing to much on my heater block now. It takes for ever to reach its temperature. I'll try and rot...

Is the extruder holder different as well? I see your extruder is more to the right.

I have added G1 Z10 to my custom GCode in slic3r to avoid that bolt when moving.

I added:
G1 Z10 ; clear bolt
G1 X10 Y10 ; move away from bolt

After that it puts the printhead to the glass and starts printing
