6 places and only 3 plugs for limit switches on main board

I have Acrylic I3 Pro B.
On the main PCB:
There are 6 sockets for limit switches, x-min, x-max, y-min, y-max, z-min, z-max.
I have only 3 limit switches, do I use the mins or the maxs?

Also; there are 4 places for fans, I have only one fan on the frame and one on the extruder, which are the correct sockets for these?

Also; schematic has extruder heater 1 and 2, I have only 1 heater, should that go into heater 1 connection and leave the other on NC?

Ditto for extruder thermistor?

One more small thing... the M4 threads in the extruder mount plate are drilled through so the M4 bolts fall straight out. I plan to re-thread these and use M5 bolts, is that OK?

And for your last question, the M5 bolt is not OK.
