You can try to upload the firmware with Easy Print or HyperTerminal,not Flash Loader.

I'm not sure about that:
It feels like so many people add extra fan, are they all soldering wires and adding power splitters ?

As you described above, 1)The heating of extruder is OK 2)Motor good 3)Cable good 4)Driver good So i assume the extruder socket on your board may be broken(no output signal) somehow... Did you fix the problem?
I guess the max current for that motor is 1.2A or 1.5A.
And i'm not sure if your extruder has the issue as the following post shows:
